ROLE OF DIABETES IN EXACERBATION OF COPD

TOPIC: Obstructive Lung Diseases TYPE: Original Investigations PURPOSE: Diabetes is a complex, chronic systemic disease, with macroangiopathic and microangiopathic end-organ complications. There emerging data that have identified high risk comorbidities for hospitalizations or readmissions, but association of diabetes Chronic Pulmonary Disease (COPD), their readmissions pulmonary function changes scarce. We sought to identify differences in rate readmission/exacerbation, effects on lung patients presenting exacerbation COPD among diabetics. METHODS: This retrospective study at Brooklyn Veterans hospital admitted the diagnosis between 2015 2016. Patients were divided into two groups: No-Diabetes. Outcomes total number admissions prior to-, 1-year post-discharge readmission Exacerbation after index hospitalization. Data was collected demographics, past medical history, length stay, testing parameters, level Hemoglobin A1C smoking history. Interstitial active cancer, ongoing infection surgery excluded. RESULTS: Of 152 identified, 76 (50%) diabetic predominantly male population both groups. had significantly higher such as hypertension (p=0.0023), hyperlipidemia (p=0.0006), Atrial fibrillation/flutter (p=0.0209) sleep apnea (p=0.0036). no difference coronary artery heart failure, deep venous thrombosis/pulmonary embolism home O2 requirement. Interestingly, rates exacerbation/readmissions DM to- (p=0.0002), after- (p=0.0168) post- (p=0.0126) admission. On multivariate analysis stood out predictor exacerbations (1-year prior-, prior- post-discharge) other comorbidities. CONCLUSIONS: conjunction has detrimental clinical course, worst outcomes increases future beyond 30-days. Higher hemoglobin creates inflammation, lowers immunity affect long term thus increasing decline. Aggressive management should be strategized early its course however larger prospective studies are required explore this observation. CLINICAL IMPLICATIONS: Early aggressive strategies implemented control Disease.
